The Minus Militia Snapback hat for Franklin, modded from the Black Snapback/Cap with the Los Santos(LS) logo on it.

Install OpenIV: https://www.gta5-mods.com/tools/openiv
Once installed, Run OpenIV and choose the game and platform.
Click on edit mode and follow the path:
OpenIV/GTA V/mods/x64v.rpf/models/cdimages/streamedpedprops.rpf/player_one_p/p_head_diff_019_l.ytd
Export the "p_head_diff_019_l.ytd" to another folder(backup of original file) before replacing it with the mod
If the system asks you to "show it in mods folder", just click on "show it in mods folder".
Player One: Franklin
Player Two: Trevor
Player Zero: I think it's Michael, I've not messed with it yet.
All my mods are for Story Mode only, I have not tried it on Online Mode.
You can actually watch this video and designed your own textures in GTA 5 using the OpenIV multi purpose editor.
